- the patient's (or guardian's for minors) driver's license number
- your insurance information

Enter “none” for driver’s license and insurance info as you can enter information and upload photos next step or the clinic can help you enter this when they call you.

Verify and/or complete Patient Information: Date of Birth if not already populated and Address
Choose a Provider either type a name or select one of the 3 options from the drop-down menu.
Choose a Pharmacy by typing pharmacy name, address or zip code and select from the results
You can skip Employer Information.
Select Communication Preferences from drop down and Yes or No for okay to leave voicemail
Enter Emergency Contact Info by entering name and phone number
Complete Form Signatures for the Notice and Consent
