- the patient's (or guardian's for minors) driver's license number
- Optionally, enter your insurance information or enter "none" for front and back this step, you can enter info later also.
Click Register
What Happens Next:
One of our staff will call to confirm your details and give you an estimated time for your telehealth visit. Keep your phone nearby!
Complete the following information while waiting for our call:
- Relationship to patient
- Insurance and Payment Information
- self pay - $50 with a credit card, add info later
- insurance -add insurance details later
- Card Images
- Scan your license, front and back - easiest to do from your phone
- Scan your insurance card, front and back - easiest to do from your phone
- Verify and/or complete Patient Information: Date of Birth and Address
- Choose a Provider either type a name or select one of the 3 options from the drop down
- Choose a Pharmacy by typing pharmacy name, address or zip code and select from results
- You can skip Employer Information.
- Select Communication Preferences from drop down and Yes or No for okay to leave voicemail
- Enter Emergency Contact Info by entering name and phone number
- Complete Form Signatures for the Notice and Consent
